The registration fee may be paid by credit card (Visa and MasterCard only!), personal check in US currency drawn on a US Bank Account, or International Money Order/Bank Draft in US currency. Checks and money orders should be payable to VITAMIN D WORKSHOP. All mailed payments must be accompanied by the on-line confirmation form which you will receive once you submit this form.

Academics and Researchers Fee: Scientists and clinicians working at universities, hospitals, non-profits, or government organizations.
Trainees Fee:
Undergraduate and graduate students and postdoctoral fellows within five years of terminal degree. A letter from the mentor confirming this status is required at the time of registration and emailed to rhall@mpi-evv.com.
Corporate Fee:
Representatives of pharmaceutical, health care and other industries (and those whose registration is sponsored by such companies).
Guest Fee:
Spouses, significant others, family members or friends of registered attendees that will not attend the scientific sessions. The Guest Fee covers the Wednesday evening Welcome Reception and the Networking Dinner Event. Name badges will be provided.

All registration categories (except Guest) include the abstract book, all program materials, entrance to general scientific and poster sessions, lunches, coffee breaks and the Welcome Reception.

For 2019, our Networking Event is a private charter cruise on the iconic Circle Line which be held on the evening of Thursday, May 30th. Cruise the Hudson and East Rivers past the Statue of Liberty and Ellis Island, under the Brooklyn Bridge and take in the spectacular New York City skyline while enjoying a four course buffet dinner including wine and beer! Adult dinner cruise tickets can be added to your registration for only $50 per person. Children ages 5 - 16 are $25 and children under age 5 are free. Pre-purchase is necessary (tickets will not be sold on-site).

The Vitamin D Workshop will print a delegate list in the scientific program book that includes your name, institution and email address.
